Cutout Placement to Align with Standoffs
One of the most common questions we get is how to account for the material thickness when trying to align say a DB-9 connector to a circuit board sitting on standoffs. Fortunately, standoffs aren't actually attached to the inside surface - they are embedded in the face. This means that if the measurement of a standoff is 1/4", then that is 1/4" from the outside of the box. When you are editing a front face cutout, if you edit the properties (right click) the y coordinate is relative to the outside of the bottom of the box. So the answer is you don't have to worry about it!

Is this still strue with the extruded sides? It looks like there is an offset in both Z and Y of say the bottom origin compared to the front origin.

it would not hold true in this case. The standoff is inserted into the bottom plate, which sits a distance above the origin of the front face.
