5 sided enclosures have an issue if the dimensions are set large enough.
Set height to be 6 inches, and set depth to be 8 inches when creating a new 5 sided enclosure. This enclosure will not display properly when set to flat.
Only seems to be an issue when going above 5" height (Which requires to increase the depth to meet the minimum of 2" larger than height on enclosures bigger than 3x3x3)

Changing the dimensions the dimensions of the 5-sided enclosure to the ones you mentioned changes the design of the base of the box from one piece to 3 pieces (base + left side + right side). When you click the 'flat' button, all the bends become flattened out. The left and right side of the box are welded to the base so they don't flatten out as if the base was one part with all sides.
